Enrollment Application

The following application must be submitted in order to be issued a schedule number by the main office for any Field Practicum course.

THE GSPH ONLINE REGISTRATION HAS BEEN CREATED ONLY FOR YOU TO OBTAIN YOUR SCHEDULE NUMBER AND ADD CODE (IF APPLICABLE) AND DOES NOT AUTOMATICALLY ENROLL YOU IN THE UNIVERSITY. AS SOON AS YOU OBTAIN YOUR SCHEDULE NUMBER AND/OR ADD CODE, YOU MUST REGISTER YOUR CLASSES THROUGH WEB PORTAL.

Before submitting the following application:

  1. Make sure you have read the Steps to Field Practice Approval.

  2. Make sure you have confered with the Field Placement Coordinator.

  3. Make sure you have already submitted the required Field Practice Forms.
