| Description: | We are looking for a Community Outreach Coordinator who will develop and maintain a thorough working knowledge of Imperial Beach Health Center services, provider, staff and funding programs. Establish and maintain a communication network in the local community. Represent Imperial Beach Health Center and its programs in the community through group and individual presentations. Will inform community members and groups about the availability of various health insuranceprograms for low-income individuals. Refer appropriate individuals to Imperial Beach Health Center for necessary medical services. This position will also assist qualified individuals in making their first appointment, if necessary. Maintain appropriate logs of all activities and meet weekly with the Executive Director or her designee to discuss outreach activities. |
| Requirements: | A working knowledge of the following programs:
- Medi-Cal for Families
- Healthy Families
- CMS
- CHDP/CHDP Gateway
- Self Pay Prenatal Care Programs/Emergency Medi-Cal
- AIM
Must be a Certified Application Assistorfor the Healthy Families program. Posses knowledge of local community and socially connected to the communities of Imperial Beach and South San Diego. Ability to communicate effectively in both English and Spanish preferred. Excellent attention to detail and able to work independently.
