You must submit a printed copy of the completed form to your faculty advisor. Once your advisor approves the form, it must be submitted to Dr. Alma Koch, Graduate Program Advisor, for approval. The entire form except signatures must be typed or computer processed. Please see your advisor if you have any questions about advancement to candidacy or how to complete the Program of Study form.
